ABOUT US

OUR MISSION

​The mission of Dr. Arenia C. Mallory Community Health Center, Inc. (Mallory CHC) is to provide high quality, customer oriented and cost-effective healthcare services in a safe and accessible environment to all persons of Holmes, Carroll, Madison, Leflore Counties and surrounding communities.

Dr. Arenia C. Mallory Community Health Center utilizes a family practice model of care to provide primary medical care to our patients.  A dedicated interdisciplinary team comprised of the medical provider, nurse, case manager, and other specialists, work together with patients to manage their health. Patients are followed closely and allowed to set obtainable health related goals with the support of a professional health care team.  Primary care services include preventive care, immunizations, and management of acute and chronic illnesses.

 

Our specialized Chronic Care Management team provides patient education and communicates with patients regarding chronic diseases such as; asthma, diabetes, hypertension, cardiovascular disease, depression and more. The providers and certified family planning counselors provide contraceptive management education about contraceptives as well as other family planning options.

 

Nutrition counseling is available to children and adults who receive primary medical care at Mallory Community Health Center.  Our nutritional counselors have expertise in prenatal nutrition, breastfeeding support, pediatric nutrition, eating disorders, weight management, and chronic diseases.  WIC services are also available on site.

OUR PROVIDERS

Providers at Mallory Community Health Center are ready to meet all of your primary care needs, from a common cold to chronic disease management. We offer convenient and accessible hours and locations.

 

At Dr. Arenia C. Mallory Community Health Center our providers are highly qualified and professionally trained.  Our medical team is comprised of family practice physicians, pediatricians, nurse practitioners, physician assistants, nurses, and medical assistants.

 

Our Dental Team includes dentists and dental assistants, along with support staff at each location.

 

Our Behavioral Health and Enabling Services team includes licensed clinical social workers, case managers and community health workers.

 

The MCHC team members play an integral role in the care of the patients and communities in which the organization serves by providing health education, helping patients navigate the health care system, and serving as healthcare advocates.

Our Services

SLIDING FEE

As a Federally Qualified Health Center, we offer sliding fee scale based on family size and income. Proof of income and family members are required by law to complete the application process.

 

The following documents are required:

1. Photo ID and address verification – one of the following

  • Current photo ID with address

  • Current photo ID with utility bill

  • Other business documents that verified your place of residency

 

2. Proof of Income

  • Prior year tax return, recent pay stubs, pensions, social security benefits, disability, veteran’s benefits, unemployment compensation, retirement, child support or alimony payments, or reference letter

  • If income is paid in cash, please provide a letter from employer to include the current date, date hired, employee’s name and address, employer’s name/address/phone number, wages per hours and how many hours of work per week, and frequency of pay period.  The letter must be signed and dated by the employee and employer.

 

3. All Household member information

 

4. Insurance card(s), including Medicaid and Medicare, if applicable

 

Sliding fee discount is calculated based on family size and income:

SERVICES WILL BE PROVIDED TO ALL PATIENTS REGARDLESS OF THEIR ABILITY TO PAY. SLIDING FEE DISCOUNTS ARE AVAILABLE BASED ON FAMILY SIZE AND INCOME. ADDITIONAL CHARGES WILL APPLY FOR LABS AND OTHER DIAGNOSTIC TESTS AT A DISCOUNTED RATE.
